Disclosures Required by the Act on the Protection of Personal Information

(1) Purpose of use of personal information and stored individual data

• To utilize for investigative purposes in order to provide, manage, and improve our services and for developing new services.
• To verify the identities of individuals when billing, processing payments, or carrying out other procedures (including the use of consumer reporting agencies for credit cards).
• To provide users with email newsletters, questionnaires, and information related to our company’s products and services.
  
  

(2) Provision of personal information to a third party

We do not provide your personal information to a third party except in the following circumstances:

1. if you provide us with consent (including cases where you authorize us to provide your personal information to a relevant organization in order for us to provide our services);
2. if it is required for the payment of fees for our services;
3. if it is required for us to provide our services;
4. if we are legally required;
   
5. if it is required to protect the life, health, or property of an individual and obtaining your consent is not possible;
6. if it is strongly necessary to improve public health or promote the healthy nurturing of children and obtaining your consent is not possible; or
7. if it is required as part of cooperating with government agencies, local public bodies, or entities acting under their purview who are carrying out duties prescribed by law, and if obtaining your consent may obstruct them from doing so.
   

(3) Procedures for disclosing, revising, deleting, and being notified of the purpose of use of personal information

1. If we receive a request from you or your legal representative asking that your personal information in our possession be disclosed, revised, deleted, no longer used, or not provided to a third party, or asking to be notified about its purpose of use (those requests collectively, “Requests”), we will respond appropriately and to an extent that is reasonable in accordance with our established procedures as well as sections 25 and 26 of the Personal Information Protection Law.

• The necessary information for making Requests is provided below. Note that we cannot respond to Requests made via methods not listed in this section, including telephone calls, emails, faxes, or visits to our company.
  
  (i) How to make Requests

  • The form prescribed by our company (see below)
    
  • Documentation verifying your identity or that of your representative (see below)
    
  • A self-addressed stamped envelope with your name and the address
    
  • Postage stamps that are equivalent in value to the processing fee for a Request for Personal Information Disclosure or Purpose of Use Notification as prescribed by our company (see below)
    
  
  Note that we cannot return any documentation sent to us as part of a Request.

  (ii) Documentation to send with your Request
  
  Required documentation varies depending on the individual making the Request.
  If making the Request on your own behalf:
  
  1. Request form (In the format specified by our company—can be downloaded below)

  •Request for Revision of Personal Information (PDF)
  •Request for the Deletion or Usage Termination of Personal Information (PDF)
  
  2. Documentation verifying your identity
  If submitting the Request yourself, please enclose one of the following valid identification
  • An original copy of your certificate of seal impression (within three months of its issue date)
  • A copy of your driver’s license
  • A copy of your passport
  • A copy of your health insurance card
  • A copy of your pension book
  • A copy of your alien registration card
  • An original copy of your certificate of residence (within three months of its issue date)
  
  If the document contains information about your permanent residence or individual number, please redact those areas before sending it to us.
  
  Note that if we cannot verify your identity from the documentation you submit, we may be unable to respond to your Request.
  
  If making the Request via a representative:
  If you are making the Request as a legal representative of a minor or adult dependent or as a proxy for the individual in question, please submit the following documentation in addition to that for the individual as listed above.
  
  3. A copy of your power of attorney affixed with the registered seal of the individual

  4. An original copy of the individual’s certificate of seal impression (within three months of its issue date)
  
  5. Documentation verifying the identity of the representative
  Please enclose one of the following valid documents
  • A copy of the representative’s driver’s license
  • A copy of the representative’s passport
  • A copy of the representative’s health insurance card
  • A copy of the representative’s pension book
  • A copy of the representative’s alien registration card
  • An original copy of the representative’s certificate of residence (within three months of its issue date)
  
  6. Verifying documentation if making the Request as a legal representative
  Please enclose one of the following to verify that you are a legal representative.
  
  If you are a parent:
  • An original copy of the individual’s koseki shōhon (within three months of its issue date)
  • A copy of your health insurance card that lists your dependents (must be valid and not expired)
  
  If you are a guardian:
  • A certificate of registered matters that lists you as a guardian (within three months of its issue date) or a copy of the court decision that names you as a guardian
  
  If the document contains information about your permanent residence or individual number, please redact those areas before sending it to us.
  If sending your koseki shōhon, please redact all information except the required items below before submitting it.
  
  Required Items: Name of the head of household; name, birth date, and parents (or foster parents) of individual, relationships
  
  Note that if we cannot verify your identity from the documentation you submit, we will be unable to respond to your Request.
  
  (iii) Processing fees and payment methods (Request for Personal Information Disclosure or Purpose of Use Notification)
  Payment of a processing fee is required when submitting a Request for Personal Information Disclosure or Purpose of Use Notification.
  
  Processing fee: ¥1,000 per Request (includes tax)
  
  Please enclose postage stamps that are equivalent in value to the processing fees corresponding to your number of Requests. If you do not enclose your payment for the processing fee or if it is insufficient, we will in most cases send a notification to the address written on the Request.
  
  Note that if we do not receive your payment within 30 days after we send that notification, your Request will be deemed invalid on the following day.
  
  (iv) Response method
  Responses will be sent via certified mail to the address of the individual listed on the Request.
  
  (v) Personal information acquired by our company through Requests
  Personal information we acquire through Requests is used only for the purpose of responding to those Requests.
  
  

(4) Consultations or complaints related to the handling of personal information

For consultations or complaints related to the handling of personal information, please submit a letter or email to one of the addresses below. Note that we cannot provide any support if you visit us in person.
